- Professional Licenses/Certificates
- Two Professional Reference Letters
- Current certification in CPR [cardiopulmonary resuscitation] and First Aid an asset within the last year
- Driver’s License/ Province ID
- Social Insurance Number
- Current Physical/PPD or chest X-ray (within the last 6 months, which states you are Free of Communicable Diseases)
- Criminal Background Check within the last two(2) years
